Assessment of combined microneedle mesotherapy with retinol and 3-O-ethyl ascorbic acid in post-acne hyperpigmentation and acne scars: the pilot case series

Summary
Introduction. Acne scars and post-acne hyperpigmentation are common skin changes that remain after the course of acne vulgaris. They may affect the appearance, structure, and tone of the skin. One of the methods used to improve the appearance of post-acne skin is microneedle mesotherapy, which may be combined with the application of active substances, such as retinol and 3-O-ethyl ascorbic acid.
Aims. The aim of this case series was to assess the effect of a series of microneedle mesotherapy treatments combined with the application of retinol and 3-O-ethyl ascorbic acid on acne scars and post-acne hyperpigmentation.
Materials and methods. This study was designed as a pilot case series. The study included four participants aged 24 years, who presented with acne scars and post-acne hyperpigmentation of varying severity. Each participant underwent a series of four treatments at four-week intervals. The effects were assessed before the start of the treatment series and two weeks after its completion. Melanin and erythema levels were evaluated using the Mexameter MX 18 probe, while skin elasticity was assessed using the Cutometer MPA 580. Photographic documentation was performed using the Fotomedicus system.
Results. After completion of the treatment series, a decrease in melanin levels was observed in most of the analyzed skin areas. Before the therapy, melanin values ranged from 100 to 198, whereas after the treatments they ranged from 81 to 193. Changes in erythema levels were more varied. Before therapy, erythema values ranged from 188 to 440, and after its completion from 232 to 471. After the treatment series, R2 parameter values increased in most of the analyzed areas, although these changes were not observed in all assessed areas. Photographic documentation indicated partial improvement in skin tone and texture.
Conclusion. The series of microneedle mesotherapy treatments combined with the application of retinol and 3-O-ethyl ascorbic acid reduced melanin levels and improved skin elasticity in most of the analyzed areas. The obtained results suggest that this therapy may support the improvement of skin appearance, particularly in relation to post-acne hyperpigmentation. The effects on acne scars were less pronounced and require further evaluation in a larger, controlled study.
